Driving Directions


Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointment please remember to obtain and bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

If you need to cancel an appointment

We understand the value of your time, and we strive to run on schedule by allotting ample time for each patient appointment by limiting the number of patients we see daily. This direct care approach allows us to give you the personalized attention and care you deserve.

If you miss a scheduled appointment without providing a 24 hours’ notice, a $50 no-show fee will be charged to your account. This fee is necessary to cover the time and resources that were reserved specifically for you.

At your first consultation, there is a direct care fee to be paid at the time of service of $250. This will include a full comprehensive history, exam and radiographs by Dr. Hobizal. This allows us to focus on proving you with the best possible care without the constrictions and limitations of insurance. From there, we will provide the best treatment plan for you and determine your coverage or provide you with transparent pricing. We will also always assist you in submitting your insurance claim to Reimbursify for self reimbursement of your visit if applicable.

we accept all major cash, check and all major credit cards. A 3% card processing fee will be added to all credit card transactions.

Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally, emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
