At Premier Foot and Ankle, we prioritize your convenience and strive to make your healthcare experience as seamless as possible. As a courtesy to our patients, we submit insurance claims to your respective insurance companies on your behalf.
Please note that while we work with most insurance providers, some of the treatments and procedures we offer may not be covered under your plan. To ensure you have all the information you need, we provide transparent pricing upfront at the time of service for any non-covered treatments or procedures.

Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.
When you come for your appointment please remember to obtain and bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)
If you need to cancel an appointment
We understand the value of your time, and we strive to run on schedule by allotting ample time for each patient appointment by limiting the number of patients we see daily. This direct care approach allows us to give you the personalized attention and care you deserve.
If you miss a scheduled appointment without providing a 24 hours’ notice, a $50 no-show fee will be charged to your account. This fee is necessary to cover the time and resources that were reserved specifically for you.
At Premier Foot and Ankle, most insurances are accepted. We do accept discounted direct payments for out of network and self pay patients.
we accept all major cash, check and all major credit cards. A 3% card processing fee will be added to all credit card transactions.
Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally, emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
